All attendees/families should complete this form.  If you currently receive financial aid or are enrolled with a community engagement partner, you will be able to submit this form without payment.

Otherwise, upon completion of this form, you will be prompted to make payment with pay pal or credit card.  If needing to pay by check or cash, please mail in payment right away.  For cash, please call the office in advance to confirm time for a visit.  508-791-8159.
